I'm John (Joshua) Millanes, a Virtual Medical Assistant and systems & automation enthusiast.
I've spent years supporting U.S.-based clinics in neurology and other specialties, helping manage the day-to-day operations that keep practices running smoothly. Along the way, I discovered that I enjoyed building systems just as much as using them.
What started as tinkering with Linux and self-hosting turned into a passion for automation, homelabbing, and software development. Today, I maintain a Proxmox homelab, build WordPress plugins and custom tools, and run a small game-server hosting business.
These days, my work sits at the intersection of healthcare operations and technology. Beyond clinical and administrative support, I create automations, integrations, and internal apps that make processes more efficient and reduce manual work.

The role where my clinical and technical skills meet. On the clinical side I handle scheduling, prior authorizations, and converting GoHighLevel leads into patients, and I led a data migration across multiple EMR systems. On the technical side I build and maintain automation workflows in GoHighLevel, wire up webhook/API integrations with Zapier and n8n, develop cross-platform and web apps, and build the WordPress plugin suites powering both platforms.
Assisted patient navigators, prepared data-analysis reports, managed medical records, served as executive assistant to clinical directors, and handled appointment scheduling and prior authorizations.
Led a remote team and oversaw patient scheduling. Coordinated with claims adjusters and nurse case managers on treatment and authorization status, handled medical records and electronic paperwork, managed inbound/outbound calls, and tracked daily work status.
Handled inbound customer concerns for major U.S. clients including Caesar's Entertainment and Target.

A monthly bill tracker and cash-flow app built with React 19 and TypeScript. Track recurring and one-time bills, see what's due, mark things paid, and get reminders before they're late. Works offline, syncs across devices, and ships as both an installable PWA and a native Android app on Google Play.
Set your payday schedule, pick how many days before each bill you want a reminder, and choose between push, email, or SMS alerts. The app handles the rest, automatically rolling recurring bills into each new month and tracking your cash flow alongside what you owe.

A game-server hosting business at scourgeservers.org. What started as hosting an Arma 3 Exile server grew into a real service: a data center in Cebu, Philippines, 10–70ms low latency for PH players, and 24/7 uptime. Hosts games like Project Zomboid and communities such as MayhemPH and The Ancient of Days.

Built with Flutter for Android and iOS, currently in active testing (TestFlight on iOS, beta on Android). It's the front door to a large community platform I also power on the web, a suite of WordPress plugins I built from scratch, with GoHighLevel automation and API integrations wiring the member workflows together.

A custom EMR integration with a private staff dashboard for clinic operations. I built a server-side integration against a FHIR API using SMART Backend Services (JWT-bearer auth, US VPN egress for compliance), exposed through a Flask API and a React dashboard that surfaces patient charts, clinical notes, and a multi-provider schedule, fully containerized with Docker.
The system is HIPAA-compliant and built to protect PHI end to end: scoped read-only access, secure session handling, and no patient data left exposed.

Most businesses that run WordPress end up stacking expensive premium plugins for features that almost fit but never quite match their workflow. This project is the opposite approach: build exactly what the client needs and nothing they don't.
I built a custom WordPress plugin suite from scratch for a client who was spending on multiple premium subscriptions. Instead of duct-taping together third-party tools with conflicting updates and licensing headaches, every feature is tailored to their exact operational workflow. Member management, content workflows, automation hooks, and integrations are all handled in-house.
The result: no recurring plugin license fees, no compatibility breakage on WordPress updates, and every feature works exactly the way the business actually operates. When they need a new capability, I build it. When something breaks, I fix it directly. No support tickets, no waiting on third-party developers.

Content submission program where members earn free membership months by submitting approved content. Handles uploads to Google Drive, Zapier integration, reviewer workflows, and WooCommerce subscription extensions.
Auto-assigns member ranks based on activity and revokes Founder status when no active subscription remains. Includes cron jobs, attendance tracking, prospect handling, and webhook notifications.
Hunt content submission form with media uploads to Google Drive and form data pushed to Zapier. Login-gated with AJAX handling and database tracking.
Member portal with announcement management. Shortcode-driven with an admin interface for managing portal content and member-facing communications.
Member connection and matchmaking for logged-in users. REST API endpoints power the matching logic with a front-end shortcode interface.
Members-only trophy photo tagging tool. Overlays a branded tag on harvest photos, submits to n8n for processing, and lets users download the result.

Controls the staff directory map: member data endpoints, business type approvals, and map display settings via AJAX-driven admin interface.
Adds a "View More Info" modal to directory member cards, showing extended profile data via AJAX. Lightweight and focused.
WooCommerce checkout overhaul. Custom-designed checkout flow that replaces the default WooCommerce checkout experience.
Exposes WooCommerce Membership user meta fields to the WordPress REST API for use with Zapier and n8n automation pipelines.
Fires a single webhook to n8n on member event updates. Clean event-driven integration for the automation pipeline.
Migration utility that applies ACF default values to users who hold an active WooCommerce Subscription. Batch processing tool.
